Title: Assessor/Case Manager - Sidney Ohio office

Hours: Part Time, 20-22.5 hours/week, daytime hours

Brief Description:

This position will be primarily assessing for PASSPORT and Assisted Living. This community-based position provides assessment, care planning and case management services for individuals from the community who are seeking nursing home placement, Community Care Choice services and/or other programs requiring a level of care determination or needs assessment. Key responsibilities include patient assessment, care plan development and implementation, ongoing case management, and consumer education.

 

Title: Assessor/Screener/PAR - Sidney Ohio office

Hours: Part-Time, 20-22.5 hours/week, daytime hours

Brief Description:

  • Completing Initial Assessments/Long-Term Care Consultations/Level of Care Assessments - primary job function
  • Providing Screening and Information and Referral Services
  • Completing Pre-Admission Review

Minimum Requirements of both:

  • Graduate of an approved school of nursing and licensed to practice in the state of Ohio as an RN
    - OR -
  • Bachelors degree in Social Work or related field and LSW
  • At least one-year prior experience in home health care, medical social work, geriatrics or an appropriate substitute as approved in advance by ODA
  • Ability to work as a team member
  • Demonstrated ability to record and provide written reports
  • Successful completion of appropriate Community Care Choice modules and certification testing

Title: Refugee Case Aid

Hours: Part-Time, 20-22.5 hours/week, daytime hours

Brief Description:
 
This part-time position will provide support services to refugee case managers to include airport pick-up, transportation to appointments, writing case notes, typing on the computer, and will provide translation and interpretation support to refugee staff.
 

Minimum Requirements

  • High school diploma or equivalency
  • Ability to be understood and understand English language; ability to write in English in a readable and concise manner
  • Proficient computer skills, including Microsoft Word and Excel
  • Driver's license and safe, personal transportation
  • Language competency in Nepali, Karen, or Arabic
