Our Northern Counties office in Sidney is our “home base” for services for older adults, including PASSPORT (Medicaid Waiver), Care Coordination and Assisted Living (waiver) in Champaign, Darke, Logan, Miami, Preble and Shelby Counties. The Sidney office also serves the Northern Counties with behavioral and mental health services as well as transportation resources.
