PASSPORT Services Help Older Adults

September 27, 2024/0 Comments/in Featured News, News /by Hope Gebhart

PASSPORT is for adults aged 60 and older who may need the level of care provided in a nursing home but hope to stay in their own homes. It is a statewide program funded by Medicaid so participants must be Medicaid-eligible. The first  step is an assessment by a nurse or social worker who determines if the older adult would qualify according to level of care requirements. The local Job and Family Services then determines Medicaid eligibility and approves enrollment.  Once enrolled, an individual is assigned a care manager who develops a care plan with customized services to help the individual remain safely at home. The care manager makes sure that needs are met with quality services and revises the care plan as needs change.

Chums Program

for older adults with diagnosed mental illness

The Chums program helps seniors (age 50 and older) with diagnosed mental illnesses to learn skills to help maintain independence and take back control of their lives.

Life Essentials staff facilitate the program by combining the practices of education, networking and behavioral activation services. The program enhances the quality of life for program members by equipping them with tools that promote self-management of their mental illness.

Participants learn common themes that contribute to their recovery including diet, exercise, goal setting, stress management and much more. They also get the opportunity to bond with people who are in similar situations.
